Prior to admission, each Hospice of the Upstate patient must meet the following criteria:

Patient is diagnosed as having a terminal illness

Certification by a medical doctor of life expectancy of six months or

less. Patients and physician should understand that prognostication

for one year or less is not an exact science.

Patient is under the care of a licensed South Carolina physician.

(If needed, Hospice of the Upstate can assist in finding a qualified

physician.)
