YOU
You are the leader of your care team.
Medical Director
In 1998, Dr. Hunter Woodall was appointed Medical Director of
Hospice of the Upstate
by the Board of Directors. In 2001, Dr. Woodall was South Carolina
board certified in
Palliative Care. He is also an AHEC Professor of Family Medicine in the AnMed Residence
Program. Dr. Woodall works closely with the Rainey Hospice
House staff
managing patient care.
Hospice Nurse
The Hospice Nurse provides skilled care and support through continuous
assessment. The Hospice Nurse is an excellent source of information
about disease processes and end of life care. The Hospice Nurse is a
direct link to the Medical Director as well as the other members of the
interdisciplinary team to provide skilled care and support tailored to the
patient’s physical condition and level of comfort. The Hospice Nurse
coordinates care of the patient and family.
Certified Nursing Assistant
The Certified Nursing Assistant (CNA) is an
expert in making daily tasks easier.
The CNA helps
maintain patient’s dignity and independence through the simple
but essential
things such as
bathing, dressing, grooming and feeding.
Pharmacist
Hospice of the Upstate has an independent pharmacy to
supply medications that are covered
by your hospice benefit. The Pharmacist is an expert in finding the
right medications to
control pain and symptoms and keep the client comfortable.
Social Worker
The Social Worker provides counseling and support
to the patient and family. Services may
include assistance in identifying community resources, advice
regarding
financial, legal and care
giving concerns that occur with a prolonged
illness, as well as other
individual services as appropriate.
Chaplain
Spirituality is generally described as an
awareness of something greater than the individual self.
The Chaplain supports your personal spiritual beliefs, practices and
needs. The Hospice of the
Upstate Chaplain makes visits, is a great listener, offers prayer, and
supports in healing
relationships. The chaplain also works with your own clergy person
and congregation as you desire.
Volunteer
The Volunteer provides companionship, practical assistance
and emotional support. Their
specific talents are matched to the clients needs. Count on them to be
willing helpers,
compassionate listeners and valuable member of your care team.
Children's Outreach Coordinator
Many times children who have experienced the
death of a loved one feel isolated from their
peers and alone in their grief. Through art, play, music, and peer support
they are given an opportunity to share their feelings and open up in an
environment that is welcoming
and understanding of their loss.
Bereavement Counselor
Bereavement services are provided to families
following the death of a patient for one year. This includes grief
counseling, support groups, telephone contacts, mailings and memorial
programs. Community members are also invited to participate in grief
support groups.
