1. Referral Center Intake Coordinator

The Referral Coordinator's is the first voice of Hospice of the Upstate. Responsibilities include delivery of extraordinary internal and external customer service with every interaction in the referral center. Experience with Medicare/ Medicaid, private insurance and hospice preferred. Ability to plan nursing visits daily and collaborate with the admission team for prompt access to care and optimum productivity. Marketing experience highly desired.

Manages all aspects of HOU referrals, preparation and scheduling including collection of accurate contact information, including patient address and expected presentation times; receives calls from referral sources, provides timely response and initiates paperwork to begin intake/referral process; provides accurate and timely information and documentation regarding HOU services; inputs HOU referrals into CareAnyware system within one hour of referral and completes first phase of admission process. 

2. Full Time- Rainey Hospice House Discharge Planner/Social Worker

Requirements preferred include a Masters of Social Work from an accredited School of Social Work, must be licensed by South Carolina Board of Social Work Examiners; active and in good standing and eligible for Georgia Social Workers license. Two years' experience in health care setting including discharge planning preferred. Assesses patients and families, develops care plans, provides counseling to patients and families and develops discharge plan for patients as needed.

3. Homecare RN

Hospice of the Upstate is seeking one full-time RN to work from 4pm to 12 midnight, Monday - Friday. The RN is responsible for identifying and managing patient and family urgent care needs in accordance with the patient's plan of care after hours. The RN responds to the patient needs by phone or making visits that include assessment and interventions. Assisting with scheduled patient requests and admissions after house is included in the job description.

4. Georgia Homecare Manager

Hospice of the Upstate is growing, and seeking a full time RN Case Manager in the Hartwell, GA area to serve patients and families in the home setting.  Current Georgia RN license required and South Carolina RN license a plus.  Prior hospice experience a plus. 5. CNA Caregiver

Hospice of the Upstate is seeking motivated, Certified Nurse Assistants to care for terminally ill patients in the home and in the Rainey Hospice House. The hospice aide is a member an interdisciplinary team and works under the supervision of a registered nurse. Hospice aides are responsible for meeting personal care needs, promoting comfort, maintaining safety, assisting with activities of daily living, and providing light housekeeping duties.

Must be a Certified Nurse Assistant, flexible and responsive in a timely manner, possess excellent communication skills, the ability to travel, a valid driver's license, excellent driving record and automobile insurance. 

      6. Home Care Case Manager -Full Time

Responsible for the management, oversight and coordination of care for assigned caseload of home hospice patients.

7. On Call Position

Ability to cover calls and visits occurring after hours and on weekends for home hospice patients.
Minimum Qualifications: Current South Carolina license as a Registered Nurse.

8. Weekend Admission Specialist - RN

The Admission Specialist presents, signs, and assesses new patient wanting to participate in the Hospice of the Upstate Program. Minimum Qualifications: Current South Carolina license as a Registered Nurse and current (or in the process of obtaining) Georgia license as a Registered Nurse

      Duties Include:
Weekend schedule
Present/Explain the Hospice of the Upstate program
Sign and assess new patients for enrollment into Hospice of the Upstate's program
Complete admissions under hospice guidelines, policy and procedure 
Participate in the Homecare on-call schedule
Assist homecare staff with initial, follow up and emergency visits as needed
Assist in the Hospice House setting as needed
Completes and charts essential communications as necessary.
