care. The successful candidates will be able to communicate
effectively with physicians and other health care staff, as well as work
well with patients, families, and staff. Accountable to maintain and
correctly document in an electronic health record. Prior hospice experience
Care Case Manager - Full time
Responsible for the management, oversight and coordination of care for
assigned caseload of home hospice patients.
Ability to cover calls and visits occurring after hours and on weekends for
home hospice patients.
Current South Carolina license as a Registered Nurse.
Admission Specialist -
The Admission Specialist presents,
signs, and assesses new patients
wanting to participate in the Hospice of the Upstate Program.
Current South Carolina license as a Registered
Current (or in the process of obtaining) Georgia license as a Registered
Hours are 8:00am
Present/Explain the HOU program.
Sign and assess new
patients for enrollment into the HOTU program.
Complete admissions under HOU guidelines, policy and procedure.
Participate in the HOU's
Assist homecare staff with
initial, follow up and emergency visits as needed.
Assist in the Hospice House
setting as needed.
Completes and charts
essential communications as necessary.
Assistant – Full Time
Assist with meal preparation, delivering and removing food
trays to patient’s rooms.
